Careteam Plus, established in 1993, is a leading provider of compassionate health care, particularly for those living with HIV in Horry, Georgetown, and Williamsburg counties. We focus on treating the whole person by identifying and addressing the root causes of poor health and collaborating with local organizations to remove barriers to quality care. Our services include adult and pediatric primary medical and specialty care, chronic disease management, infectious disease prevention, weight loss and smoking cessation assistance, medical referrals, nursing care coordination, and case management services.
